Q: What information will a psychologist need for my child's appointment?
 

A: You will need to provide copies of all previous psychological, educational, and medical assessments that your child had before that and a copy of your child's current Individual Educational Plan (IEP) or 504 plan. You may be asked to complete parent's questionnaires and to have your child's teachers complete questionnaires. Since the psychologist will ask you questions about your child's medical history, early development, social history, and school history, you will need to bring to the appointment any information that will help you answer these questions.
